Issues need to be analysed for root cause, and the cause addressed within 7 days in order to improve the performance of the project team. Good practice is to do this as a team in an open and constructive manner.
A good way to determine root cause is to ask "Why did this happen?" 5 times. You are normally not dealing with root cause until you are talking about processes and people. @patrewin
